การเปรียบเทียบการวัดความหนาตรงกลางกระจกตาด้วย Anterior Segment Optical Coherence Tomography และ Ultrasound Pachymetry ในผู้ใหญ่สุขภาพตาปกติ

การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบค่าความหนาของกระจกตาส่วนกลาง (Central Corneal Thickness; CCT) เมื่อวัดด้วย Ultrasound pachymetry (US pachymetry) ด้วยเครื่อง Tomey SP-100 และ Anterior Segment Optical Coherence Tomography (AS-OCT) ด้วยเครื่อง Cirrus HD-OCT 6000
การศึกษาครั้งนี้เป็นการศึกษาเชิงพรรณนาภาคตัดขวาง (Cross-sectional study) ในกลุ่มตัวอย่างจำนวน 78 ราย (156 ตา) ที่มีอายุตั้งแต่ 18 ปีขึ้นไป โดยวัด CCT ในผู้เข้าร่วมแบบไม่สัมผัสด้วย AS-OCT และแบบสัมผัสด้วย US pachymetry โดยดำเนินการวัดข้างละ 3 ครั้ง และนำค่าเฉลี่ยมาวิเคราะห์ทางสถิติ โดยใช้ paired t-test หรือ Wilcoxon signed-rank test ตามการแจกแจงของข้อมูล และในการวิเคราะห์ค่าความต่างของ 2 เครื่องมือด้วย Bland-Altman Analysis นอกจากนี้ได้วิเคราะห์ความสัมพันธ์ระหว่างค่า CCT ที่วัดได้จากเครื่องมือทั้งสองชนิดโดยใช้ Pearson Correlation Coefficient และรายงานค่าสัมประสิทธิ์สหสัมพันธ์ (r) และค่า p โดยกําหนดค่านัยสําคัญทางสถิติที่ p < 0.05
จากผลการศึกษาพบว่า กลุ่มตัวอย่างจำนวน 78 ราย (156 ตา) มีค่าเฉลี่ย CCT ที่วัดด้วย US pachymetry เท่ากับ 536.70 ± 31.20 µm ในตาขวา และ 536.20 ± 30.48 µm ในตาซ้าย ขณะที่การวัดด้วย AS-OCT เท่ากับ 526.20 ± 28.92 µm และ 525.60 ± 29.42 µm ตามลำดับ โดยค่า CCT ที่วัดด้วย US pachymetry มีค่าสูงกว่า AS-OCT อย่างมีนัยสำคัญทางสถิติ (p < 0.001) และมีค่าเฉลี่ยความแตกต่างเท่ากับ 10.51 µm ผลการวิเคราะห์ Bland-Altman แสดงให้เห็นว่าทั้งสองเครื่องมือมีความสอดคล้องกันในระดับปานกลาง และผลการวิเคราะห์เพิ่มเติมด้วย Pearson correlation coefficient แสดงความสัมพันธ์เชิงบวกระดับสูง แต่ยังมีความแตกต่างของค่าที่วัดได้ สรุปได้ว่า การวัด CCT ด้วย US pachymetry และ AS-OCT ให้ค่าที่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ ซึ่งไม่สามารถทดแทนกันโดยตรงในทางคลินิก อย่างไรก็ตาม ทั้งสองวิธีสามารถใช้ติดตามผลผู้ป่วยได้ หากใช้เครื่องมือชนิดเดียวกันอย่างสม่ำเสมอ
